Transform Diabetes Care - Join the Team to empower our residents.We have a rewarding opportunity to work alongide a highly dedicated team to deliver outstanding care to residents across Doutta Galla's eight aged care homes.In this role you will be responsible for providing expert clinical advice and assisting in the provision of high quality and individualised diabetes management for residents/participants and their families living with,or at risk of diabetes.What you'll bring:Dedication to resident care and supportAt least five years experience as a Registered Nurse (Div 1)Experience as a Diabetes Educator and working with Diabetes technology Post Grad qualification in Diabetes EducationExcellent leadership capability to support diabetes education, management and care initiativesExperience in promoting resident / participant chronic diseases self-managementPlan, co-ordinate and implement Clinical Teaching Programmes underpinned by Diabetes Australia guidelinesAbility to implement change management initiatives and projects as required to support the organisation in achieving compliance with legislative requirementsExcellent interpersonal, written and verbal communication skills Ability to effectively communicate with residents, resident representatives and other health professionalsA good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to clinical careThe Organisation:Doutta Galla Aged Services is a not for profit aged care organisation with over thirty years’ experience caring for people from a diverse range of backgrounds.
